The Johnstown Walk of Hope started in 2016 with the goal of supporting local patients with a cancer diagnosis. The money raised through the walk supports patient expenses that include medical testing, treatment, travel, medications, and some household expenses. The mission of the JWOH is to lessen the financial burden so that patients can concentrate on staying strong and getting well.

The purpose of this form is to nominate patients for support who are receiving their cancer treatment outside of the Cambria-Somerset area. Patients may be referred by an individual or an agency outside of the JWOH partnership agreement.
